Continuity of Care ProgramProvides financial assistance to supplement support services for families of children with severe disabilities and/or high medical needs requiring intensive in-home support.
Flexible RespiteThe respite program provides financial assistance for families with children and young people 6 to 18 years old with disabilities. Brokerage money is available to purchase a range of respite options where other respite services are not available for the family. Support is offered on a short-term basis only.
