Stepped Care for Older Adults is a free mental and physical wellbeing support service for older adults.
The older person may be experiencing anxiety and/or depressive symptoms, social isolation, loneliness, and/or other psychosocial stressors.
They may be at risk of developing mental illness or currently have a mild to moderate mental illness.
The program consists of a triage service, allied health mental and physical health assessment, and evidence-based person-centred interventions in-line with the principles of recovery oriented practice. Stepped Care provides both home visits and centre based appointments.
Aged care facility residents are eligible and the service welcomes the CALD and LGBTIQA + community. We accept self-referrals or referrals from health professionals. Please note this is not a crisis service.
Eligibility:
Living in Brimbank, Melton, Darebin, and Moreland
Adults 65 years and older (50 years and older for Aboriginal and Torres Strait Islander people)
Interested in improving their wellbeing
No mental illness diagnosis required
